When you visit the Centre for the 1st treatment:

• Dress comfortably (as you will be at the Centre for at least 4 hours)
• Bring along the following:
  • Memo from the Hospital
  • Hospital Discharge Summary
  • Last 3 haemodialysis treatment records, if any
  • List of all your medications
  • Medisave balance statement
  • Insurance Policy and Plan
  • Blanket (as you will be in an air-conditioned place for many hours)

Please arrive at the centre 15 minutes before the scheduled time of treatment.  Notify the staff when you arrive. We will first orientate you to the centre and its surroundings.
